And The Real Reason You Feel Like a Different Person

It's 3:07 AM.

You're wide awake.

Heart pounding. Mind spinning. No reason. No threat. Just your brain doing whatever it wants while your body lies there completely exhausted.

Tomorrow is already on your chest and it hasn't started yet.

And you are lying in the dark, genuinely wondering if you are losing your mind.

You're not. But I know that doesn't feel true at 3AM.

I also know you probably haven't told anyone how bad it's gotten. Because you're the one who handles things. You always have been.

The Woman I See Every Week

She's the most capable person in every room she walks into. And she comes to me because something is off and she cannot figure out what.

She's exhausted in a way sleep doesn't fix. Snapping at people she loves. Her brain, the sharp and reliable brain she has built her whole life on, is suddenly foggy. She's gained weight in her midsection despite doing everything right. The anxiety is new and confusing.

What's Actually Happening (The Science Nobody Explains)

Perimenopause can begin in your late 30s. Your period may barely change. What shifts first is how you feel, think, and sleep. Here's why.

Progesterone drops first. It metabolizes in the brain into allopregnanolone, a natural sedative that binds to GABA-A receptors, the same receptors targeted by anti-anxiety medications. When progesterone declines, that buffer disappears. Cortisol has less opposition. Your nervous system becomes reactive. That 3AM wake-up with the racing heart? That is your brain without its natural regulator.

Estrogen doesn't just decline. It swings. In early perimenopause, estrogen can spike two to three times higher than normal before crashing. Estrogen directly influences serotonin and dopamine signaling. Unstable estrogen means unstable mood and cognition, which explains why some days feel fine and others feel completely unmanageable.

Insulin sensitivity drops 15 to 25 percent. Estrogen helps regulate glucose uptake at the cellular level. When it fluctuates, metabolism shifts. Weight migrates to the midsection. Cravings increase. And no amount of clean eating moves the needle the way it used to, because the metabolic environment has fundamentally changed. This is also why tools like microdosing GLP-1, when used thoughtfully inside a full hormonal protocol, can be a meaningful piece of the picture for some women.

Here is something I see constantly and cannot stay quiet about.

 The women who need support the most are the ones least likely to ask for it.
 
Not because they don't deserve it. Not because they don't know something is wrong. But because their entire identity is built around being the one who handles it. Who figures it out. Who does not complain. Who keeps going no matter what.

And perimenopause walks right into that dynamic and exploits it completely.
